Issue Description
If you have multiple 1004MC forms, 1004D forms, or USPAP Identification Addenda in your report, none of them will display properly in the OADI Viewer.
Instructions
To ensure that these forms render properly in the OADI Viewer, you must remove any duplicates from your report. In other words, you can have one of each of these forms in your report, but you cannot have more than one 1004MC, etc.
